Please include a copy of the member’s birth certificate.
Note: We only need the birth certificate on new members. Renewals do not need to resubmit one every year.Â

You may complete the online application, scan and email your printed application and documentation to [email protected] , OR stop by the Club and a representative will contact you regarding your application.
You can complete the form and return to the office at the Club or email your form at [email protected]. We also have printed forms available at the Club.
Update parent/guardian and/or emergency contact information for your member.
Form allows the Club to distribute medications to your member while at the Club.
