If you are interested in becoming a member or would like to renew your membership, we would love to have you!
You can apply online or download our application, print and fill out the form, and then submit to us by email or at the Club. Please follow the instructions below and note that all new members and parent(s) must attend an orientation meeting before attending The Club For Boys.

Please include a copy of the member’s birth certificate.
Note: We only need the birth certificate on new members. Renewals do not need to resubmit one every year.Â

You can complete the form and return to the office at the Club or email your form at [email protected]. We also have printed forms available at the Club.

Important medical information regarding, food allergies, medications, medical or physical limitations and other conditions.
Transportation Form for the member to include transportation from school to the Club with information regarding rules and procedures.
